Examples of Documentation for 67% Completion and/or GPA
(These are SOME examples that can be turned in; not ALL examples)
- ER/Hospital documentation
- Police Report
- Employment verification and/or work schedule changes (in writing)
- Signed by supervisor and on letterhead
- Rehabilitation paperwork and/or Counselor report
- Death Certificate and/or obituary
- Make sure documentation is included to show the relationship to the deceased family member
- Birth Certificate
- Statement from a 3rd party**
- Unemployment paperwork
Examples of Documentation for 150% Maximum Timeframe
- Proof of a previously earned degree
- Why your first degree didn’t help you, and why your current degree will.
- ER/Hospital documentation
- Police Report

- Rehabilitation paperwork and/or Counselor report

** A 3rd party can include, but is not limited to, a statement on letterhead from a supervisor, clergy-person, counselor, psychiatrist, or medical professional who supports your claim. The 3rd party statement cannot be from a family member.
