Examples of Documentation for 67% Completion and/or GPA

(These are SOME examples that can be turned in; not ALL examples)

• ER/Hospital documentation 
• Police Report 
• Employment verification and/or work schedule changes (in writing)
• Signed by supervisor and on letterhead
• Rehabilitation paperwork and/or Counselor report
• Death Certificate and/or obituary
• Make sure documentation is included to show the relationship to the deceased family member
• Birth Certificate
• Statement from a 3rd party**
• Unemployment paperwork 

Examples of Documentation for 150% Maximum Timeframe

• Proof of a previously earned degree
• Why your first degree didn’t help you, and why your current degree will. 
• ER/Hospital documentation
• Police Report 
• Employment verification and/or work schedule changes (in writing)
• Signed by supervisor and on letterhead
• Rehabilitation paperwork and/or Counselor report
• Death Certificate and/or obituary
• Make sure documentation is included to show the relationship to the deceased family member
• Birth Certificate
• Letter from a 3rd party**
• Unemployment paperwork 

** A 3rd party can include, but is not limited to, a statement on letterhead from a supervisor, clergy-person, counselor, psychiatrist, or medical professional who supports your claim. The 3rd party statement cannot be from a family member.
