Financial Aid Forms and Documents

You are only required to submit documents that are listed in your myFSCJ To-Do List. Your completed documents can be submitted to the Financial Aid Office (FAO) by fax to (904) 633-5958, email to financialaid@fscj.edu, hand delivery to your nearest Advising Center or mail to FSCJ at the following address:

NOTE: Students who are required to create an Inceptia account will be notified through their myFSCJ portal via their ToDo list. If you are chosen for verification and asked to create an account with Inceptia, the documents requested through Inceptia must be submitted through the verification gateway (Inceptia Portal).

Tips

  1. Make sure that your document or form is clear and legible – no coffee stains or scribbles!
  2. Double check that your document or form contains all of the appropriate signatures.
  3. Is your document or form for the correct year? Most financial aid information is year-specific.
  4. Keep copies of everything that you submit.
  5. Within a few days of submitting your information, check your myFSCJ account to confirm that it has been updated as received.
  6. Check your FSCJ email and myFSCJ accounts frequently so that you are always up-to-date on your financial aid!
