In compliance with Florida Statute 119.071(5), Florida State College at Jacksonville issues this notification regarding the purpose of the collection and use of Social Security numbers. Florida State College will collect your Social Security Number (SSN) if specifically required by law to do so or if necessary for the performance of the College's duties and responsibilities. The College takes appropriate measures to secure SSNs from unauthorized access and does not release SSNs to other parties except as required to fulfill the College's duties and responsibilities. A student may refuse to disclose his or her SSN to the College, but the IRS is then authorized to fine the student in the amount of $50. Florida Residency for Tuition Purposes

  • Residency Affidavit (PDF) - A student who is seeking classification as a Florida resident (in-state) for tuition purposes must complete a residency affidavit as a part of the admission process. The residency affidavit must be received no later than the first day of the session/term in which residency is sought. Students wishing to provide additional information to support a request for Florida residency may submit a supplemental form (PDF) with the residency affidavit.
  • Request for Residency Reclassification (PDF) - A student who is classified as a non–Florida resident (out-of-state) and seeks reclassification as a Florida resident (in-state) must complete the residency reclassification form and submit with all supporting documentation prior to the first day of the first session/term in which residency is sought.

  • Readmission Application (PDF) - Use this application if you are applying for readmission (12 or more consecutive months of non-enrollment).
