Special Note Regarding 2013-14 Forms

There are many changes to the verification process for the 2013-2014 academic year. The College is still hard at work updating the verification forms for the 2013-2014 year and making the final changes.

If your FAFSA was selected for verification, you will not be able to access the verification forms at this time. While we are finishing our part, please make sure that you have completed the following steps to make the process as easy as possible:

  1. If you have not already done so, please use the IRS data retrieval process within the FAFSA. Using the IRS data retrieval process is the best (and easiest) way to ensure the accuracy of the information on your FAFSA.

  2. If you were unable to use the IRS data retrieval tool (or chose not to), please obtain a copy of your (and your parents or your spouse's as applicable) 2012 federal income tax transcript and wage transcript from the IRS. You will need to submit the tax transcripts to the College with your verification form when it is available.

Clarification Statement/Worksheet 
Federal regulations require financial aid staff to review and correct all conflicting information indicated on the Student Aid Report (SAR). The financial aid checklist available in Connections provides a detailed explanation of section(s) on this form that must be completed to clarify conflicting information.
Clarification Statement - Low Income and Assets
Federal regulations require that we check the accuracy of the information you provided. You have submitted zero income or a very low income, based on the information you have provided thus far, we need clarification in order to verify your student aid application.
Dependent Verification Worksheet
Some dependent financial aid applicants are required to submit verification of income and their parents’ income. Submit this form along with any required tax returns. Be sure to include all required signatures.
Independent Student Status Documentation
When you completed the 2011-2012 FAFSA, you answered “yes” to one of the questions that could make you independent for financial aid purposes. Please complete this form (printing it in color is recommended) and attach the required documentation so that we can determine your status.
Independent Verification Worksheet
Some independent financial aid applicants are required to submit verification of their income. Students who are married, must also include their spouse income information. Submit this form along with any required tax returns. Be sure to sign the form.
Loan Request Form
The loan request form is used by students to have a loan added to their financial aid package or to request an increase to an existing loan. Please complete all sections of the form and submit to your campus financial aid office.

Parent Statement
The parent statement is used to clarify information reported on a student’s FAFSA related to his/her parent’s name, date of birth or social security number.
Direct Loan Entrance Counseling
Before a first-time William D. Ford Federal Direct Loan (DL) Program borrower takes out a loan (subsidized/unsubsidized), the school must ensure that entrance counseling is conducted. Loan counseling addresses the importance of repayment and managing debt, as well as other details regarding a loan. To complete the process you will need the Federal Student Aid Pin you used to electronically sign your FAFSA.

IRS Withholding Documentation
The IRS Withholding Document is required for students who are not U.S. citizens or Permanent Residents that are receiving financial aid, scholarships or grants.
Financial Aid Authorization
The Financial Aid Authorization Form gives Florida State College permission to deduct miscellaneous fees and prior debts from financial aid funds.
Thank You Letter
Recipients of Florida State College Foundation scholarships are required to send a thank you letter to the person(s) who have funded the scholarship.

Commonly Requested Documents/Documentation

  • Athletic Aid Form: This form is submitted by the athletic department on behalf of the student.
  • Birth Certificates: The financial aid office may request a copy of your or your dependent’s birth certificate.
  • Default Document: The National Student Loan Data System (NSLDS) indicates that you are in DEFAULT on one or more federal student loans. You are not eligible to receive any federal student aid until you resolve any loan default(s). To resolve your defaulted federal student loan(s), contact the lender associated with the loan and/or the Department of Education at 1-(800) 621-3115. Once you have resolved the default situation, you must provide documentation from your lender or the Department of Education to your campus financial aid/enrollment services office.
  • Free Application for Federal Student Aid (FAFSA): Apply online and Florida State College will receive your information electronically.
  • Honors Contract: Students awarded an honors scholarships must return a signed Honors Contract to the scholarship department.
  • Income Tax Return and W-2s: If you were unable to use the IRS data retrieval tool (or chose not to), please obtain a copy of your (and your parents or your spouse's, as applicable) 2012 federal income tax transcript and wage transcript from the IRS (www.IRS.gov or call 1-(800) 908-9946). You will need to submit the tax transcripts to the College with your verification form.
  • Items Related to Proof of Financial Aid Independent Status: Students who are under the age of 24 may be required to submit proof of their independent status. Examples of independent proof are marriage certificate, child’s birth certificate, DD214 (U.S. military standard separation document), etc.
  • Items Related to Immigration Status: If your FAFSA did not match with the Department of Homeland Security, you may be required to submit additional immigration documents. Examples, passport, resident alien card, etc.
  • National Letter of Intent: This form is submitted by the athletic department on behalf of the student.
  • Scholarship Letter of Acceptance: This letter is mailed with certain scholarship award packages. Please return your Letter of Acceptance to the Scholarship department.
  • Selective Service: If you are required to provide proof that you registered with Selective Service, visit the Selective Service System website.
  • Social Security Benefits Statement: To request a Replacement SSA-1099, visit the Social Security website.
  • Social Security Card: The financial aid office may require a copy of your or your parent’s social security card.
  • Talent Grant Agreement: This agreement is submitted by the talent grant administrator on behalf of the student.
